Job summary

Qualifications

• Master’s Degree in Social Work from an accredited institution.
• Eligible for licensure as LLMSW or LMSW by the Michigan Board of Social Work.
• Minimum of 2 years of social work experience in a healthcare setting.

Responsibilities

• Provide social work services during the 8:00 AM – 4:30 PM shift in the inpatient hospital on the weekends only.
• Conduct psychosocial assessments for patients and families in crisis.
• Develop treatment and discharge plans addressing emotional, social, and practical needs.
• Deliver brief counseling and crisis intervention.
• Coordinate with community agencies and internal resources to support safe care transitions.
• Act as patient advocate and liaison between care teams and families.
• Document assessments, plans, interventions, and outcomes timely and accurately.
• Collaborate with the interdisciplinary care team for holistic patient care.
