Job summary

Qualifications

• BS Physical Therapy from an accredited program.
• Active MA Physical Therapy license OR candidates for licensure by exam.
• American Heart Association – BLS certified.

Responsibilities

• Examines patient using appropriate testing methods and records findings, and identifies needs.
• Develops a detailed assessment and treatment plan with input from patient/family and other health professionals; ensures goals are functional, time-based, and measurable.
• Safely implements treatment interventions with sound clinical judgment and patient cooperation.
• Provides patient/family education during therapy and discharge planning.
• Documents discharge summary and maintains medical records per policy.
