Job summary

Trio Health Partners is seeking a Weekend In-House Nurse Practitioner to support the primary provider of internal medicine services within a Skilled Nursing Facility. You will develop strong relationships with residents, families, facility staff, and providers, and play a leadership role in clinical care.

The ideal candidate holds an active Illinois NP license and a Master's in Nursing, with a background in geriatric/long-term care settings.

Qualifications

Active Illinois NP license (unencumbered).
Master's Degree in Nursing from an accredited program.
Strong clinical background in primary care and preventive medicine.
Geriatric or adult medicine experience in skilled nursing / long-term care / hospice settings preferred.
Ability to engage and communicate with elderly and chronically ill patients and families.
Ability to maximize onsite time to develop robust caseloads.
Knowledge of Medicare, Medicaid, and Health Plan benefit structures is a plus.

Responsibilities

Ensure continuity of care by communicating with the primary In-House NP.
Conduct new admission evaluations and monthly stability visits for long-term residents.
Perform comprehensive physical examinations and evaluate changes in patient conditions.
Prescribe medications and treatments as needed.
Develop patient care policies and facilitate referrals to specialists.
Coordinate advanced care planning and discharge visits.
Ensure timely communication and documentation of patient assessments and follow-ups.
Support the physician-patient relationship and address gaps in care.
