Job summary

Northern Illinois Hospice is seeking a compassionate Hospice Social Worker for weekend shifts in Illinois. You will provide psychosocial support to patients and families and collaborate with the care team to guide them through the hospice journey.

Requirements include a Master’s in Social Work (MSW), IL LSW or LCSW preferred, 1+ year experience in healthcare or hospice, a valid driver's license, and the ability to respond within 30 minutes of your assigned area.

Qualifications

• Must have MSW and IL LSW or LCSW preferred.
• 1+ year experience in healthcare or hospice (preferred).
• Valid driver’s license and reliable transportation.
• Local residence with ability to respond within 30 minutes of assigned service area.

Responsibilities

• Complete psychosocial assessments in hospice care.
• Develop care plans and provide counseling as a Social Worker.
• Coordinate community resources and referrals.
• Support advance care planning (DPOA-HC).
• Maintain timely, accurate documentation.
• Collaborate with the interdisciplinary hospice team.
• Ensure compliance with Medicare and Illinois standards.
