Job summary

Enhabit Home Health & Hospice is seeking a licensed Master of Social Work (MSW) professional to administer home-based social services and support to patients and their families in Illinois. The role requires licensure in the state, at least one year of healthcare social work experience, and comfort with mobile tech tools.

The selected candidate will collaborate with physicians and care teams to implement the plan of care and educate families, enabling high-quality care in the home setting.

Qualifications

• Must possess a master's degree from a school of social work accredited by the council on social work education.
• Must be certified or licensed in the state where they currently practice.
• Must have at least one year of social work experience in a health care setting.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Administer social services to patients requiring intermittent professional services and teach the patient, family, and other members of the health care team.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).
• Provide education and support to patients and families to optimize care at home.
