Lighthouse Hospice Care LLC is seeking an on-call Weekend Hospice Registered Nurse to provide compassionate nursing care in patients' homes and facilities in Downers Grove, IL.

Shifts run Saturdays 8am-8pm and may include on-call duties. You will assess patients, manage symptoms, administer medications, educate families, document care in EMR, and collaborate with the interdisciplinary team to support end-of-life care.

Competenze

• Graduate from an accredited Nursing program (Diploma/ADN/BSN).
• Current RN license and valid driver's license.
• Hospice experience preferred; strong communication and palliation skills.

Mansioni

• Provide on-call hospice nursing care in response to patient or family needs during assigned shift.
• Perform patient assessments, manage symptoms, and administer medications as needed.
• Collaborate with the interdisciplinary team (physicians, social workers, chaplains, hospice aides, etc.).
• Educate patient and family regarding disease process, self-care, end-of-life care, nutrition, and dietary needs.
• Maintain accurate documentation of care provided using EMR.
• Communicate changes in patient condition to the care team and adjust care plans accordingly.
• Provide emotional support to patients, families, and caregivers.
• Ensure interventions adhere to hospice policies and regulatory guidelines.
