Qualifications

• CPR certification and continuing education requirements.
• Willingness to travel and cover weekend/evening/holiday on-call shifts.

Responsibilities

• Care and documentation meeting reimbursement guidelines.
• Coordinate physical care of the patient with family, patient, physician and Hospice team.
• Communicate with patients, families, and agency personnel.
• Attend IDT meetings and quality reviews as scheduled by agency.
