Job summary

Hartford HealthCare at Home is seeking a Registered Nurse to support patients and families at home, delivering compassionate end-of-life care. You will perform comprehensive assessments, set goals for symptom management, and collaborate with an interdisciplinary team to address holistic patient needs.

The role requires CT RN licensure, at least 1 year of RN experience (homecare or hospice preferred), and a valid driver’s license with reliable transportation.

Qualifications

• Registered Nurse with valid CT RN license.
• One year of RN experience; Homecare or Hospice experience preferred.
• Valid driver’s license and reliable transportation required.
• Computer proficiency required.

Responsibilities

• Support our patients, families, and loved ones through their end-of-life journey.
• Perform a comprehensive assessment to establish goals for symptom management and end-of-life care with effective case management strategies.
• Be a member of an active interdisciplinary team that addresses patients’ needs holistically.
• Develop and maintain a therapeutic relationship through effective communication with patients and their families.
• Utilize state-of-the-art technology to document skilled services provided.
