The Care Team Hospice is seeking a Hospice Registered Nurse (RN) Weekend Warrior to join our team in Gaylord, Michigan. This full-time position features manageable caseloads and flexibility in scheduling, offering coverage from Friday to Monday, with options to adjust hours.

Qualifications

• Active Registered Nurse (RN) license required.
• Hospice or home health experience preferred.
• Ability to work independently in the field.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized plans of care.
• Provide skilled nursing care in accordance with hospice standards.
• Coordinate care with physicians and interdisciplinary team.
• Educate patients and families on symptom management.
• Document care plans accurately in the EMR.
• Support after-hours patient needs as part of on-call rotation.
