Job summary

St. Croix Hospice is seeking a Registered Nurse On-Call/Visit Nurse to support our 24/7 care model on weekends. The role involves responding to urgent patient needs, completing admissions, and guiding families through end-of-life care with compassion and expertise.

Requirements include a state-licensed RN, 1+ year acute care experience (hospice/home care preferred), and current licensure. A valid driver's license and reliable transportation are essential for this on-site weekend position in

Qualifications

Graduate of an accredited nursing program (Associates, Bachelors, or Masters).
1+ years of recent acute care experience required; hospice or home care experience required.
Current RN license in state of practice.
Respond to triage calls and provide nursing visits for symptom management, admissions, and care plan updates.
Develop and adjust individualized care plans based on evolving needs.
Administer medications and treatments as prescribed, offering education and support to patients and caregivers.
Maintain timely and accurate clinical documentation of care, outcomes, and communication.
Must have reliable transportation, valid driver’s license, and proof of auto insurance.
