Job summary

Advocate Health is seeking a Hospice Registered Nurse (RN) to provide after-hours hospice care across Milwaukee South and surrounding areas. The role includes comprehensive nursing assessments, triage, symptom management, and education for patients and families.

Travel to patients' homes is required, with coordination of care within the interdisciplinary team. The ideal candidate will have an active RN license in Wisconsin, BLS certification, and about one year of hospice or related clinical

Qualifications

• Completion of an accredited or approved nursing program.
• Active Registered Nurse (RN) license in the state of practice.
• Basic Life Support (BLS) certification – American Heart Association.

Responsibilities

• Provide comprehensive nursing assessment, triage, and intervention for hospice patients during after-hours shifts.
• Respond to patient and caregiver calls to assess symptoms and determine appropriate interventions.
• Conduct in-person visits to manage acute symptom changes and ensure patient comfort and safety.
