Job summary

Agape Care Group is seeking a Team Coordinator for our Lexington Community Hospice House. This part-time weekend role involves leading and serving, handling calls, scheduling, and supporting clinicians to ensure patients receive timely care.

You will work with a supportive leadership team and have opportunities for professional development and advancement. The role emphasizes organizing, inventory management, and ensuring smooth daily operations.

Qualifications

• A heart to serve patients and families and a passion for providing the best possible care
• Education: Associate degree preferred
• Experience: Minimum of 1-year experience in a health care setting. Scheduling and office manager experience preferred
• Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 25 lbs

Responsibilities

• Answer all incoming calls and manage daily workflow
• Assignment of clinician visits and scheduling
• Preparing interdisciplinary meetings and supporting the clinical leader(s)
• Managing and maintaining office inventory and medical supplies
• Ensure the smooth operation of the agency so patients can receive the care they need

Education
