We are seeking an experience hospice RN or LVN to work Thursday 5pm-Monday to work Triage.

The Weekend RN / LVN will be responsible for providing care and services for a group of hospice patients, and visiting patients where they reside, which could include private homes, assisted living facilities, and hospitals.

As an RN / LVN, you will complete hospice admissions and patient visits as needed. Applicants must have great clinical and communication skills. The Weekend RN / LVN is responsible for answering calls from patients, families and customers. The RN / LVN will make nursing visits for symptom management, deaths, and admissions as needed. This RN / LVN position must also be passionate about hospice care and be a great advocate for both hospice services and Suncrest Hospice.

• Current Licensed Vocational Nurse (LVN) or Registered Nurse (RN) licensure (in good standing) in the state(s) of practice with one year of experience under license
• Admissions and On-call experience preferred
• Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Work closely with patients on a 1-1 level but get strong support from a patient care team.
• Can work independently and on your own on a day-to-day basis with activities that include phone, email, and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations.
• Time management skills to manage visits, documentation, and meetings (i.e. schedule visits, attend required meetings, and respond to urgent needs by prioritizing and triaging multiple concerns)
• Ability to learn an electronic medical records system.
• Must be willing to drive with reliable transportation, a valid driver’s license, and auto insurance, to be able to travel across the service area
