Job summary

Bridge Home Health and Hospice in Murrieta, CA is seeking a Licensed Vocational Nurse for Saturday and Sunday shifts (12a-8:30a) to provide direct patient care under RN supervision and participate in care coordination.

The role requires an LPN/LVN license in CA, CPR certification, a clean driving record, and strong communication skills. Prior home health experience is preferred and patient care plans will be implemented per the RN's guidance.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Three years nursing experience preferred (home health or medical/surgical).
• Currently licensed as a practical nurse or vocational nurse in California.

Responsibilities

• Provides direct patient care as defined by the state Nurse Practice Act.
• Implements plan of care initiated by the RN.
• Documents accurately and timely per the plan of care.
• Educates patient and family/caregiver on self-care and diagnosis.
• Coordinates home health services with other disciplines and RN/Clinical Manager.
• Prepares and uses equipment per aseptic technique.
• Engages in ongoing professional growth and development.
• Performs other duties as assigned by the RN.
