Job summary

Talented Medical Solutions is seeking a Licensed Vocational Nurse (LVN) for Home Health Hospice in Sonora, CA. This contract role requires 40 hours/week across day shifts with weekend work and three weekdays, starting 08/31 for a 13-week assignment.

Responsibilities include direct patient care under RN/MD supervision, participating in care planning, and administering medications with on-call duties as needed. State licensure and BLS certification are required, with valid DL preferred.

Qualifications

• Licensed Vocational Nurse (LVN) licensure in CA is required.
• BLS Health Care Provider certification is required.
• Bachelor's Degree or equivalent preferred (if applicable).
• Arrhythmia certification is preferred.

Responsibilities

• Provides care for assigned patients under the supervision of an RN or MD according to nursing process.
• Performs routine visits based on policy and procedures.
• Assists in development of an interdisciplinary plan of care.
• Implements and coordinates plan of care using organizational skills.
• Implements teaching plans and discharge plans with patient involvement.
• Performs clinical procedures (phlebotomy, injections, drug screens, etc.) and administers medications.
• Orders/restocks supplies and maintains cleanliness in assigned areas.
• Performs other job-related duties as assigned.
