Job summary

Brighton Hospice is seeking a Weekend LPN to provide needs-based visits to patients in their homes across the community, including private residences, assisted living and SNFs. You will also answer calls from patients and families and coordinate with the care team.

Requires current LVN/LPN licensure in the state, strong clinical and communication skills, and a passion for hospice care. Weekend shifts include 8- or 12-hour options, with opportunities to grow within a national company.

Qualifications

• Current LVN/LPN licensure (in good standing) in the state of practice.
• Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Work closely with patients on a 1-1 level but get strong support from a patient care team.
• Can work independently and on your own from a day-to-day basis with activities which include: phone, email and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations.
• Time management skills to manage visits, documentation, meetings (i.e.: to scheduled visits, attend required meetings, respond to urgent needs by prioritizing and triaging multiple concerns)
• Ability to learn an electronic medical records system.
• Must be willing to drive with reliable transportation, Driver's License, Car insurance

Responsibilities
