Job summary

UnitedHealth Group's Home Health of Wilson, part of the Optum family, seeks a Registered Nurse to provide and direct nursing care in patients' homes on a Fri-Sun schedule, with full-time benefits and a $10k sign-on bonus.

The RN uses OASIS, coordinates care with the team, supervises aides, documents per policy, and participates in quality and safety initiatives to reduce avoidable hospitalizations.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Valid Driver’s License, vehicle insurance, and access to a dependable vehicle or public transportation.

Responsibilities

• Develops and updates patient plans of care with physicians and interdisciplinary team; completes comprehensive nursing assessments (including OASIS and medication reconciliation).
• Provides and coordinates high-quality clinical care; supervises/home health aides and LPN/LVNs per regulations.
• Documents all patient care per policy; communicates with physicians, patients, families, and staff for coordinated care.
