Job summary

Optum in New Jersey is seeking a Registered Nurse in Home Health to provide and direct nursing care to patients in their homes, coordinating with physicians and the care team to ensure continuity of care. This full-time role includes a $5,000 sign-on bonus for external candidates and a comprehensive benefits package.

Requires RN licensure, 1+ year home health experience, CPR certification, and a valid driver’s license; you will develop and manage care plans, supervise staff, and document care

Qualifications

• Current and unrestricted RN licensure in state of practice.
• 1+ years of Home Health experience
• Current CPR Certification
• Valid Driver's License and reliable vehicle or public transportation

Responsibilities

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals
• Supervises care provided by home health staff and assigns tasks per regulations
• Completes comprehensive assessments (OASIS) including medication reconciliation
• Documents patient visits per policy and payer requirements and coordinates with the health care team
