Valley Regional Home Health is seeking a part-time or full-time Registered Nurse to join a growing team delivering in-home care. RNs will work Saturdays and Sundays after training, with initial training Mon-Fri.

After independence, schedules may be adjusted to patient needs, visiting 3–5 patients daily within the Valley area. We emphasize employee growth and a supportive culture; a locally owned organization since 2013, offering a broad benefits package and a dedicated nursing team for community

Competenze

• Must hold current RN licensure in California.
• Graduate of NLN-accredited nursing program.
• Maintain current CPR certification.
• Must have a valid driver''s license with insured vehicle.
• One year of home health experience preferred but not required.

Valley Regional Home Health is currently seeking a part-time or full-time Registered Nurse who would be able to work Saturdays and Sundays after training. Please note: Applicants must be able and willing to train Monday through Friday for at least one month. Training hours are generally 8:00am-4:30pm, but once the employee is independent, RNs can adjust their working days, start times, and end times to provide patient care in the home environment. Our home health RNs see between 3-5 patients per day and drive between patient visits.

Patient Care

• Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es). Ability to read documentation on patient status as well as any orders that the patient PCP may have provided.
• Regularly re-evaluates patient nursing needs and completes necessary charting within the required timeframe.
• Initiates the plan of care and makes necessary revisions as patient status and needs change; uses health assessment data to determine nursing diagnosis.
• Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
• Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications, treatments, IV ABX, etc. as prescribed by the physician.
• Counsels the patient and family in meeting nursing and related needs, provides education on plan of care
• Provides health care instructions to the patient as appropriate per assessment and plan of care.
• Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.

Position Qualifications

• Registered nurse with current licensure to practice professional nursing in the state.
• Graduate of National League for Nursing accredited school of nursing.
• Maintains a current CPR certification.
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order - as well as willing to drive around the job posting area.
• Demonstrates excellent observation, problem solving, verbal and written communication skills; ability to work well with others and communicate time-sensitive information effectively.
• Shows ability to organize and prioritize workload independently – home health employees are typically independent in the field.
• One year of home health experience preferred, but not required.
