Job summary

Valley Regional Home Health is seeking a full-time Registered Nurse to serve the Tulare, Visalia, Kingsburg, Selma, Dinuba and Hanford areas. Training hours are 8:00am-4:30pm; once independent, you may adjust start/end times for in-home patient care.

RNs typically see 3–5 patients per day, driving between visits. We’re a locally owned agency focused on employee growth, benefits, and community service.

Qualifications

• Must hold a California RN license.
• Graduate of NLN-accredited nursing program.
• Current CPR certification.
• Valid driver’s license with insured vehicle; willing to drive in service area.
• Excellent communication and ability to work independently.

Responsibilities

• Complete initial assessment of patient and family for home care.
• Re-evaluate patient needs and document in charting within required timeframes.
• Initiate and revise the plan of care based on patient status.
• Develop care plans with goals and nursing actions; involve patient and family.
• Administer prescribed medications and IV antibiotics as ordered.
• Counsel patient and family on plan of care and education needs.
• Identify discharge planning needs and address before discharge.

Job description

Valley Regional Home Health is currently seeking a full-time Registered Nurse for the Tulare, Visalia, Kingsburg, Selma, Dinuba, and Hanford areas. This position would be Monday through Friday. Training hours are generally 8:00am-4:30pm, but once the employee is independent, RNs can adjust their start times and end times to provide patient care in the home environment. Our home health RNs see between 3-5 patients per day and drive between patient visits.

Patient Care

• Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es). Ability to read documentation on patient status as well as any orders that the patient PCP may have provided.
• Regularly re-evaluates patient nursing needs and completes necessary charting within the required timeframe.
• Initiates the plan of care and makes necessary revisions as patient status and needs change; uses health assessment data to determine nursing diagnosis.
• Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
• Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications, treatments, IV ABX, etc. as prescribed by the physician.
• Counsels the patient and family in meeting nursing and related needs, provides education on plan of care
• Provides health care instructions to the patient as appropriate per assessment and plan of care.
• Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.

Position Qualifications

• Registered nurse with current licensure to practice professional nursing in the state.
• Graduate of National League for Nursing accredited school of nursing.
• Maintains a current CPR certification.
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order - as well as willing to drive around the job posting area.
• Demonstrates excellent observation, problem solving, verbal and written communication skills; ability to work well with others and communicate time-sensitive information effectively.
• Shows ability to organize and prioritize workload independently – home health employees are typically independent in the field.
• One year of home health experience preferred, but not required.
