Job summary

Halcyon Home LLC in Austin, TX is seeking a Weekend Home Health Registered Nurse (RN) on a pay-per-visit basis to support our geriatric patient population in the home. The role focuses on compassionate, patient-centered care for seniors receiving home health services.

You will perform initial and ongoing assessments, educate patients and families, plan and coordinate care with the IDG, and ensure documentation and quality care in a home setting.

Qualifications

• Current RN license in good standing.
• Home health or related setting experience preferred.
• Strong clinical and assessment skills.
• Excellent communication and interpersonal abilities.

Responsibilities

• Initial and ongoing comprehensive assessments of patient needs in home health.
• Educate patient and family regarding disease process, self-care, and care requirements.
• Determine scope and frequency of services based on acuity and needs.
• Initiate preventive and rehabilitative nursing procedures.
• Provide clinical directions to hospice aides and LPN/LVN.
• Prepare progress notes and ensure proper documentation of care.
• Inform physician and team of changes in patient needs and outcomes.
• Promote agency philosophy and ensure continuity of quality care.
