Job summary

Halcyon Home is seeking a dedicated Home Health Registered Nurse PRN Pay Per Visit to support our geriatric population in Lakeway, TX and surrounding areas. This flexible PRN role involves approximately 5–10+ visits per week with opportunities throughout weekdays and weekends.

Qualifications

• Current RN license and in good standing with the state nursing board.
• Willingness to complete 5–10+ visits per week on a flexible PRN schedule, including weekends as needed.
• Experience in home health or related healthcare setting preferred.
• Knowledge or experience with Homecare Homebase EMR is a plus.
• Strong clinical and assessment skills.
• Excellent communication and interpersonal abilities.
• Ability to work independently and manage time effectively.

Responsibilities

• Perform initial and ongoing comprehensive assessments of patient needs.
• Develop, implement, and revise the plan of care for quality and continuity.
• Document progress and communicate changes to the physician and care team.
• Provide clinical directions to care staff to ensure service quality.
• Ensure accurate documentation and regulatory compliance.
• Educate patient and family about disease process, self-care, and end-of-life care.
• Coordinate with and inform the IDG about patient needs and responses.
