Job summary

Aveanna Healthcare is seeking a Weekend Baylor Registered Nurse to provide in-home nursing care in the Athens area. You will conduct in-home assessments, deliver skilled nursing interventions, and coordinate with physicians to ensure high-quality outcomes.

Qualifications

Holds an active RN license in the state of practice with at least one year of clinical nursing experience.
Current driver's license, reliable transportation, and auto insurance.
CPR certified (or able to obtain certification).

Responsibilities

Provide one-on-one skilled nursing care in the home to promote recovery, safety, and independence.
Perform comprehensive in-home assessments and translate findings into patient-centered care plans.
Deliver wound care, infusions, catheter care, medication management, postoperative and chronic disease management.
Educate patients and caregivers, guiding symptom management and daily home care.
Monitor patient progress, communicate changes to the care team, and adjust plans to prevent hospitalizations.
Coordinate care across providers for continuity and quality outcomes.
Complete timely documentation reflecting the clinical picture and outcomes.
