Job summary

UnitedHealth Group is seeking a Weekend Registered Nurse in Home Health to deliver and direct nursing care in patients' homes, coordinating with physicians and the health-care team to ensure continuity of care.

This full-time position in Madison, OH involves supervising home health aides, performing supervisory visits, and delivering high-quality services within infection-control standards. OASIS assessments and comprehensive nursing visits are required.

Qualifications

• Active RN licensure in the state of practice.
• Experience in home health is preferred.
• Current CPR certification or ability to obtain within 90 days.

Responsibilities

• Initiate, develop, implement, and revise the plan of care with physicians and team.
• Supervise care by home health aides and licensed nurses.
• Provide required supervisory visits.
• Deliver high-quality clinical services within scope and infection control standards.
• Complete comprehensive assessments (OASIS) and reconcile medications accurately.
• Document patient visits per policy and payer requirements.
• Make initial and comprehensive nursing evaluation visits for eligibility.
• Communicate timely with agency staff and physicians.
• Participate in orientation of new staff and act as a preceptor.
• Participate in on-call and weekend rotation as needed.
