Qualifications

• Registered Nurses (RNs) with strong assessment, skilled nursing and documentation skills are strongly encouraged to apply.
• Must be able to check email intermittently during the week to respond to documentation requests.
• RNs must be okay with traveling throughout the service area.

Responsabilités

• Provide top quality patient care using an individualized care plan.
• Visit 6-8 patients on weekends, driving to their homes.
• Conduct initial assessment and case development for patients.
