Job summary

CHRISTUS HomeCare - Tyler, part of the LHC Group and Optum family, seeks an experienced RN for home health. You will provide and direct nursing care to patients in their homes and coordinate the plan of care with the physician and care team.

The role includes supervising home health staff, ensuring accurate documentation and OASIS assessments, and participating in on-call rotations. Current RN licensure and driver’s license required.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current driver's license and access to a dependable vehicle or public transportation.
• CPR certification or ability to complete within 90 days of hire.
• 1+ years of Home Health experience.
• Ability to work independently.
• Solid communication, writing, and organizational skills.

Responsibilities

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals.
• Supervises care provided by home health aides and licensed practical/vocational nurses, provides instruction, and assigns tasks according to State and federal regulations.
• Provides required supervisory visits.
• Documents patient visits and completes comprehensive assessments (OASIS) accurately and timely.
• Ensures patients meet home health eligibility and medical necessity guidelines and submits accurate documentation.
• Communicates with physicians, patients, and family to ensure quality care and service excellence.
• Takes direction from Clinical/Executive Directors and participates in survey readiness and performance improvement activities.
• Participates in on-call and weekend rotation as needed.
