Responsibilities

• Deliver skilled, patient-centered care in a home health setting, collaborating with an interdisciplinary team
• Perform patient assessments, medication management, and care planning
• Educate patients and families on disease management and treatment plans
• Maintain thorough, compliant EMR documentation
• Work independently, but collaborate with team as needed
• Travel to patient homes within assigned territory
