Concierge Home Care is hiring a Weekend Home Health RN (RN) in the Orange Park area. This modified full-time role features a 4-day schedule (Friday–Monday) with 1:1 patient care and full autonomy.

Qualifications

• Active, unencumbered Florida RN license.
• Valid driver’s license and reliable transportation.
• Home health experience preferred but not required.

Responsibilities

• Perform comprehensive patient assessments in the home setting.
• Develop and manage individualized care plans.
• Provide skilled nursing care and patient/caregiver education.
• Administer treatments and monitor patient condition.
• Coordinate care with physicians and interdisciplinary teams.
• Evaluate patient progress and adjust care as needed.
• Complete accurate, timely documentation using streamlined tools.
