Job summary

BAYADA Home Health Care is seeking a weekend Physical Therapist (PT) to perform home health visits for our Denver office. The role focuses on Start of Care/Admissions and requires Medicare Certified Home Health and OASIS documentation experience.

Per diem, part-time, and full-time scheduling options are available, with incentives for weekend commitments and productivity targets. Travel throughout the Denver coverage area is required.

Qualifications

• Must hold a CO PT license.
• Minimum 1 year of PT work experience.
• Graduate from a program approved by APTA or AMA Commission on Allied Health Education and Accreditation.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, gait, strength, balance, safety).
• Implement wound care and dressings.
• Develop and implement individualized care plans with PT interventions.
• Continually assess and revise the PT care plan with nursing in the multidisciplinary team.
• Educate clients and families on rehabilitative care and activities for health and independence.
• Accurately document observations and interventions in EMR.
