Job summary

BAYADA Home Health Care is seeking a weekend Physical Therapist (PT) to perform home health visits for our Denver office. The role focuses on Start of Care/Admissions and requires Medicare Certified Home Health and OASIS documentation experience.

Per diem, part-time, and full-time scheduling options are available, with incentives for weekend commitments and productivity targets. Travel throughout the Denver coverage area is required.

Qualifications

• Must hold a CO PT license.
• Minimum 1 year of PT work experience.
• Graduate from a program approved by APTA or AMA Commission on Allied Health Education and Accreditation.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, gait, strength, balance, safety).
• Implement wound care and dressings.
• Develop and implement individualized care plans with PT interventions.
• Continually assess and revise the PT care plan with nursing in the multidisciplinary team.
• Educate clients and families on rehabilitative care and activities for health and independence.
• Accurately document observations and interventions in EMR.

BAYADA Home Health Care is seeking an experienced Physical Therapist, PT for an opportunity performing home health visits for our Denver office.

This is a Weekend Physical Therapist position - as visits will include mostly Start of Care / Admissions, Medicare Certified Home Health and OASIS documentation experience is required.

Per Diem, Part Time, and Full Time scheduling options, including:

• $5,000 Incentive if able to commit to 4 SOC per weekend (2 Saturday + 2 Sunday) OR
• $5,000 Incentive if able to commit to 20 productivity points per weekend (combination of Fri, Sat, Sun, and/or Mon) - benefits eligible!

As a home care Physical Therapist, you will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home. Please note that this role requires flexibility and the ability to travel throughout the office's entire coverage area.

Responsibilities for a PT - Physical Therapist include:

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
• Implement integumentary protective interventions to prevent wound formation and promote wound repair utilizing evidence-based dressings and topical agents.
• Develop and implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
• Continually assess and revise the Physical Therapy - PT care plan, and participate with nursing in the multidisciplinary care plan, as appropriate.
• Educate and instruct clients, family members, or other client representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet.

Qualifications for a PT - Physical Therapist include:

• A current CO Physical Therapist - PT license.
• A minimum of one year of recent work experience as a Physical Therapist - PT.
• Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.

Additional Information:

• Base Pay: $65-70 per point, depending on qualifications
• Mileage reimbursement
