Job summary

Adoration Home Health in Denver, CO is seeking a dedicated Home Health Physical Therapist for a PRN weekend schedule. You will perform 1:1 patient care, evaluations and treatments per physician orders, and develop individualized plans while ensuring thorough documentation for billing.

Qualifications

• Bachelor’s Degree in Physical Therapy from an accredited program.
• At least 2 years of PT experience; hospice/community experience preferred.
• Valid license to practice Physical Therapy (state of practice).
• Current CPR certification.

Responsibilities

• Perform PT evaluations and treatments per physician orders, including adaptive equipment.
• Establish measurable goals and treatment plans to maximize independence and function.
• Maintain communication among the treatment team, physician, patient, and family.
• Maintain thorough documentation for proper billing.
• Supervise PT Assistants and PT Aides in direct patient care per state act.

Responsibilities

• Perform physical therapy evaluations and treatments based upon physician orders, including recommendation and procurement of adaptive equipment
• Establish measurable goals and develop treatment plans to move patients toward their maximum level of independence and/or function
• Maintain communication among the treatment team, physician, patient, and family members as applicable
• Maintain thorough and timely documentation of services provided to ensure proper billing
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with state practice act

Qualifications

• Minimum of a Bachelor’s Degree in Physical Therapy from a college or university with an accredited Physical Therapy program
• New physical therapy graduates may be considered in select markets based on program availability
• At least (2) years of appropriate experience as a physical therapist; community/hospice experience is preferred
• Valid license from the state of practice
• Possess and maintain current CPR certification

Salary Range

USD $58.00 - $60.00 / Hour
