BAYADA Home Health Care is currently seeking an experienced Weekend Physical Therapist to provide exceptional care to adult and geriatric patients in their homes and in Milford, Medway, Franklin and Surrounding Towns. Prior home health experience is strongly preferred but not required. As a home health physical therapist, you will be an integral member of a multidisciplinary healthcare team that provides skilled nursing and rehabilitative care, enabling clients to maintain their independence.

Responsibilities include:

1. Making visits to clients in their homes
2. Performing diagnostic tests and measurements, such as mobility/range of joints, transfer status, stability, ambulation patterns, strength and endurance of muscles, balance testing, and safety assessments.
3. Developing and implementing individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
4. Assessing and revising the Physical Therapy care plan and collaborating with nursing in the multidisciplinary care plan.
5. Educating and instructing clients, family members, or other representatives in rehabilitative care and activities to promote health, safety, and independence.
6. Accurately documenting observations, interventions, and evaluations using a state-of-the-art touch pad tablet.

Qualifications include:

• A current license as a Physical Therapist in Massachusetts
• A minimum of one year of recent clinical experience as a licensed Physical Therapist
