Job summary

Community Home Health Care & CIHC seeks an experienced Registered Nurse (RN) to join our home health team for weekend coverage (Saturday and Sunday). The RN will oversee patient care, support caregivers, ensure quality care, and help coordinate services for patients in their homes.

You will visit 8–10 patients per day on weekends, document care accurately, and work with families, physicians, and the care team to improve patient outcomes.

Qualifications

• Active Indiana RN license.
• Minimum 2 years of home health nursing experience.
• Valid driver's license and reliable transportation.
• Strong assessment, communication, and organizational skills.
• Experience using EMR/EHR.
• Bilingual candidates encouraged.

Responsibilities

• Visit patients in their homes and monitor the quality of care.
• Assess patient needs and develop plans of care.
• Support and guide caregivers and clinical staff.
• Communicate with patients, families, physicians, and the care team.
• Ensure documentation is accurate, complete, and submitted on time.
• Identify and resolve patient or staff concerns.
• Follow company policies and state and federal regulations.
• Use sound nursing judgment to solve problems and improve patient outcomes.
