Job summary

Compassus in Nashville, TN seeks a Clinical Care Partner III to coordinate safe, efficient transitions of care for hospitalized patients, evaluating for home-based services and supporting discharge planning in collaboration with physicians, case management, families, and post-acute providers.

This full-time weekend role requires residing locally and may follow a 5x8h Thursday–Monday or 4x10h Friday–Monday schedule, with a focus on reducing length of stay, readmissions, and ensuring patients

Qualifications

• Education: An accredited degree in nursing (RN), Master of Social Work (LCSW), or Master’s degree in Speech Language Pathology.
• Preferred: 2–3 years of experience in care coordination, discharge planning, or healthcare services.
• Experience with EMR systems (Epic) and referral platforms.
• Licensure: Active RN, LCSW, or SLP licensure and CPR certification.

Responsibilities

• Coordinate safe, efficient transitions of care and discharge planning.
• Evaluate patients for home-based and post-acute care services.
• Arrange post-acute services including home health, hospice, DME, medications, and follow-up care.
• Educate patients and families and coordinate with hospital staff and post-acute partners.
