Qualifications

• Required: Associate’s degree in Nursing, Health Sciences, or related field.
• Preferred: Bachelor’s degree in nursing, Health Sciences, or related field.
• Active LPN/LVN license and CPR certification are required.

Responsibilities

• Coordinate safe, timely discharge planning and transitions for hospital patients.
• Evaluate suitability for home-based and post-acute care services.
• Educate patients/families on post-acute options and care expectations.
• Manage referrals and maintain relationships with hospital and post‑acute providers.
• Support KPI tracking and care coordination strategy.
