Job summary

Apex Home Health, Hospice & Home Care is seeking a Certified Nurse Assistant (CNA) for home care services with weekend availability. The role involves direct patient care, personal hygiene assistance, mobility support, and meal preparation in clients' homes.

You will document care using EVV and maintain patient confidentiality while following all safety protocols. Role requires DFS certification, good communication, and physical ability to lift and assist with transfers.

Qualifications

• DFS-certified CNA (current certification)
• High school diploma or GED preferred
• Effective communication, reading and writing skills; ability to follow instructions
• Physical ability to lift and stand for extended periods

• Assist with transfers, repositioning, and ambulation to promote comfort and safety
• Prepare meals considering client preferences and special diets
• Offer companionship and engage in activities tailored to each patient
• Perform light housekeeping and maintain accurate care records
• Monitor patient condition and report changes per care plan; document using EVV systems
• Maintain confidentiality, respect privacy, and follow safety protocols including reporting abuse or neglect
