Job summary

Sage Creek Post-Acute is seeking a Driver-CNA for weekend shifts to provide routine nursing care to residents according to care plans. You will identify residents, document care, and report any changes to supervising nurses, while maintaining strict confidentiality and working within facility policies.

The role involves assisting residents with daily activities, documenting patient data, and collaborating with nursing staff to ensure high-quality care on weekend shifts.

Qualifications

• Must possess a minimum 10th grade education.
• Licensed Certified Nursing Assistant in accordance with state law.
• CPR certification preferred.

Responsibilities

• Provide routine daily nursing care and services per resident assessment and care plan.
• Identify residents before administering treatments or meals using wristband/photo ID.
• Report changes in resident condition to Nurse Supervisor/Charge Nurse promptly.
• Record entries on flow sheets, notes, and charts in a clear, descriptive manner.
• Maintain confidentiality of resident information and report any unauthorized disclosures.
