Responsibilities

• Ensures a safe, supportive environment under direction of charge nurse or supervisor.
• Assists patients with activities of daily living.
• Performs safety checks and monitors patient conditions.
• Records vital signs and communicates changes to the clinical team.
• Engages patients in activities and supports treatment goals.
• Assists with transfers, charting, and incident reporting.
