Job summary

Innovive Health is seeking a Weekend Baylor RN to provide one-on-one skilled nursing in patients' homes, helping them maintain independence with high-quality care.

This full-time, exempt role offers autonomy, team support, and meaningful patient relationships; weekend schedule includes Saturday and Sunday 7:00 AM–8:00 PM with a Monday follow-up window.

Qualifications

• Active RN license required.
• BSN with 1+ year of experience or ASN/Diploma with 2+ years experience.
• Current CPR certification.
• Valid driver’s license and reliable transportation required.

Responsibilities

• Perform patient assessments and develop individualized care plans.
• Deliver skilled nursing care, including medication and treatment management.
• Monitor patient progress and adjust care as needed.
• Educate patients and caregivers on health management and self-care.
• Coordinate with physicians and interdisciplinary teams.
• Supervise Home Health Aides and LPNs.
• Complete timely and accurate clinical documentation (including OASIS).

Education

RN license

Weekend Baylor Registered Nurse (RN) – Home Health

Our Weekend Baylor RNs provide one-on-one skilled nursing care in the home, helping patients maintain independence while receiving high-quality, personalized care. This role offers autonomy, strong team support, and meaningful patient relationships.

Schedule (Full-Time, Exempt)

• Saturday & Sunday: 7:00 AM – 8:00 PM
• Monday: 4 hours for follow-ups and documentation

Key Responsibilities

• Perform patient assessments and develop individualized care plans
• Deliver skilled nursing care, including medication and treatment management
• Monitor patient progress and adjust care as needed
• Educate patients and caregivers on health management and self-care
• Coordinate with physicians and interdisciplinary teams

Physical Demands & Work Environment

• Frequent standing, walking, sitting, and use of hands
• Regular bending, kneeling, and reaching
• Occasional lifting and repositioning of patients
• Ability to lift/push up to and over 25 lbs as needed
• Exposure to clinical environments and infectious materials
