Job summary

Bridgeway Home Health is seeking a Weekend Baylor Registered Nurse to perform skilled home health visits on Fri/Sat/Sun with 12-hour shifts or Fri/Sat/Sun/Mon with 10-hour shifts in Rockdale, Newton, Henry counties and surrounding areas in Georgia.

The Baylor RN delivers autonomous, high-quality care, educating patients and coordinating with the care team to ensure timely documentation and positive outcomes. Active RN license and CPR required, with home health experience preferred.

Qualifications

• Active RN license in good standing.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and auto insurance.
• Home health experience required.
• OASIS experience required.

Responsibilities

• Complete skilled nursing visits in patient homes.
• Perform patient assessments and ongoing evaluations.
• Administer treatments and provide disease management education.
• Monitor medication compliance and patient progress.
• Coordinate care with physicians, therapists, caregivers, and interdisciplinary team members.
• Accurately complete electronic documentation in a timely manner.
