BAYADA Home Health Care is seeking a Registered Nurse (RN) for home health in Lansdale, PA. This full-time weekend Baylor RN position serves adult and geriatric clients across Montgomery County and parts of Philadelphia, with Friday-Sunday or Saturday-Monday shifts.

Qualifications

• Active PA RN license.
• Minimum 1 year clinical experience.
• Prior home care preferred but not required.

Responsibilities

• Follow designated care plans per patient needs.
• Perform home visits to clients in assigned geographies.
• Administer medications, wound care, treatments, and procedures.
• Monitor patient conditions and report changes to supervisor.
• Document doctors' orders and client assessments.
• Coordinate case management and communication with care team.
