The Palms Home Care is seeking an experienced Home Health RN for a full-time, salaried Weekend Baylor position in Florida. The role covers Friday through Monday with a productivity expectation of 32 points per week.

Responsibilities include skilled visits, assessments, medication management, education, and care coordination with the clinical team. OASIS experience and WellSky/Kinnser familiarity are preferred.

Qualifications

• Active Florida RN license and CPR certification.
• Home health nursing experience with OASIS proficiency.
• Strong clinical judgment, communication, and time-management skills.
• Ability to work independently within the assigned territory.

Responsibilities

• Perform skilled nursing visits and comprehensive assessments (OASIS start-of-care, soc, recert, discharge).
• Provide medication management, wound care, and patient/caregiver education.
• Develop and update individualized plans of care with physicians and clinical team.
• Monitor patient condition changes and communicate concerns promptly.
• Coordinate care with patients, families, and other healthcare professionals.
• Complete accurate, timely documentation in WellSky/Kinnser.
• Meet the weekly productivity requirement of 32 points.

Schedule: Friday - Monday | Productivity: 32 points per week

The Palms Home Care is seeking an experienced home health RN for a full-time, salaried Weekend Baylor position. This role provides skilled nursing care Friday through Monday, with a productivity expectation of 32 points per week.

Responsibilities

• Perform skilled nursing visits and comprehensive assessments, including OASIS start-of-care, resumption-of-care, recertification, and discharge assessments.

• Provide medication management, wound care, and patient and caregiver education.

• Develop and update individualized plans of care in collaboration with physicians and the clinical team.

• Monitor changes in patient condition and promptly communicate concerns.

• Coordinate care with patients, families, and other healthcare professionals.

• Complete accurate, timely documentation in WellSky/Kinnser.

• Meet the weekly productivity requirement of 32 points.

Qualifications

• Active Florida RN license and current CPR certification.

• Previous home health nursing experience and OASIS proficiency.

• Strong clinical judgment, communication, and time-management skills.

• Ability to work independently within the assigned territory.

• Valid driver’s license and reliable transportation.

• WellSky/Kinnser experience preferred.
