BridgeWay Home Health in Conyers, GA is seeking an experienced Home Health RN for a Weekend Baylor schedule that offers Friday through Sunday shifts, with a 12-hour day option, or Friday through Monday with 10-hour days. Enjoy consistency, flexibility, and better work-life balance.

You’ll perform skilled nursing visits in patients’ homes, complete assessments, implement care plans, and coordinate with physicians and the interdisciplinary team.

Qualifications

• RN license in good standing required.
• Home health experience required.
• OASIS experience required.
• CPR certification current.
• Valid drivers license with reliable transportation.
• Excellent clinical assessment and organization.

Responsibilities

• Provide skilled nursing visits in patients' homes.
• Perform comprehensive patient assessments and evaluations.
• Develop and implement care plans.
• Provide treatments and disease-management education.
• Monitor medications and patient progress.
• Educate patients and caregivers on disease processes and self-care.

Weekend Baylor Schedule

Option 1 – 3 Days:
Friday Saturday Sunday
12-hour day shifts

Position Summary

As a Weekend Baylor RN, you will provide skilled nursing care to patients in their homes while helping them achieve the best possible clinical outcomes.

You’ll have the opportunity to build meaningful relationships with patients and families while working independently and collaborating with physicians and the home health interdisciplinary team.

• Develop and implement individualized plans of care
• Provide treatments and disease-management education
• Monitor medications, symptoms, and patient progress

• Communicate and coordinate care with physicians, therapists, caregivers, and other members of the interdisciplinary team
• Recognize changes in patient condition and communicate appropriately
• Complete accurate and timely electronic clinical documentation
