Job summary

Palms Home Care in Florida is seeking an experienced Registered Nurse for a Weekend Baylor position, Friday through Monday. This role offers independence, flexible scheduling, and meaningful one-on-one home health care.

You will perform skilled visits, assessments, care plans, wound care, med management, and documentation, coordinating with physicians and the care team while maintaining strong patient education.

Qualifications

• Active and unrestricted Florida RN license.
• At least one year of nursing experience.
• Medicare-certified home health and OASIS experience strongly preferred.
• Current CPR certification.
• Strong assessment, communication, and time-management skills.
• Ability to work independently and manage a weekend caseload.
• Valid driver’s license, reliable transportation, and current automobile insurance.

Responsibilities

• Provide skilled nursing visits to home health patients Friday through Monday.
• Complete patient assessments and develop individualized plans of care.
• Perform admissions, Start of Care, resumptions of care, recertifications, routine visits, and discharges as assigned.
• Provide wound care, medication management, disease education, and postoperative support.
• Monitor patients’ conditions and promptly communicate changes to physicians and the clinical team.
• Educate patients and caregivers on medications, treatments, safety, and disease management.
• Coordinate care with physicians, therapists, caregivers, and other members of the interdisciplinary team.
• Complete accurate and timely clinical documentation.
• Assist with urgent weekend patient needs within the assigned territory.
