Position Summary

The Weekend Baylor Home Health Registered Nurse is responsible for comprehensive nursing assessments, planning, and care to optimize patient comfort and health in their home environments. This includes patient teaching aligned with their needs and the interdisciplinary care plan. The RN practices in accordance with the NH State Nurse Practice Act. This full-time, exempt role requires availability from Friday at 4:30 pm through Monday at 8:00 am, including on-call duties and home visits, primarily based in Littleton and surrounding areas.

Education

  • Graduation from an accredited school of professional nursing.

Licensure

  • Current RN license in New Hampshire.
  • Basic Life Support (BLS) certification from an accredited course, or Heartsaver CPR AED, with renewal on a regular basis and a grace period of up to three months after expiration.

Skills

  • Ability to work independently in a home care setting.

Work Experience

  • Previous clinical experience is preferred but not mandatory.

  • Collaborate with healthcare teams for coordinated care.
  • Document assessments, interventions, and responses accurately.
  • Manage multiple visits and on-call duties efficiently.
  • Provide emotional support respecting patient values.
  • Adjust care plans based on patient feedback and conditions.
  • Ensure compliance with NH State Nurse Practice Act and policies.
  • Maintain clear communication with patients, families, and teams.
  • Respond to emergencies promptly and adapt care accordingly.
  • Conduct health education sessions for patients and families.
  • Engage in ongoing professional development.
  • Use resources efficiently to support care and operations.
