21st Century Home Health Services (21HHS) in the Sonoma area is seeking a Part-time Registered Nurse Case Manager to assess patients and coordinate comprehensive home health care. You will perform SOC admissions, follow-ups, and visits across the service area, documenting care and coordinating with the medical team.
You will need a CA RN license, BLS/CPR certification, a CA driver’s license, and strong clinical skills.
21st Century Home Health Services (21HHS) is the fastest-growing home health agency in Northern California. We care for patients across San Francisco, San Mateo, Santa Clara, Santa Cruz, Alameda, Contra Costa, Solano, Napa, Yolo, Placer, El Dorado, Sacramento, Marin, and Sonoma counties.
There's a reason for that growth. Words like collaboration, teamwork, and productivity stay buzzwords unless the structure, model, and tools make them real. At 21HHS, we've built a model that combines the best of home health with the structure, resources, and support clinicians need to thrive. Our clinical guiding principles are simple: empower, enhance, educate.
When clinicians get real support and structure, the results show up in two places at once: patient outcomes and employee satisfaction. We know our clinicians and therapists provide outstanding care because our patients, their families, and their caregivers tell us so. Check out our Yelp and Google reviews. We're proud of this work.
We've also set a new benchmark for employee satisfaction in home health. As a Top Workplace, 21HHS supports growth and recognition through open communication and professional development. Feedback from our team on Glassdoor and Indeed backs this up. Our outcomes, low turnover, rapid growth, and high morale speak for themselves.
