Job summary

21st Century Home Health Services (21HHS) is seeking a Part-time Registered Nurse Case Manager to cover the Marin Area. You will perform SOC admissions, conduct follow-ups, and visit patients in their homes across designated territory.

Qualified candidates will hold a CA RN license, BLS/CPR, and a CA driver’s license. Experience in home health, high acuity patients, and IV therapy is highly desirable.

Qualifications

• Current and unrestricted CA RN license.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Experience with Start of Care (SOC) - OASIS is highly desired.
• High level of clinical knowledge in home health care is highly preferred.
• Experience with high acuity patients is a plus.
• Experience working with an interdisciplinary team is highly desired.
• Case Management fundamentals is highly desired.
• IV therapy initiation experience is a plus.

Responsibilities

• Conduct Start of Care (SOC) Admissions - OASIS.
• Follow up visits and evaluations in designated territory.
• Make home visits to patients in designated territory.
• Reconcile medications, wound care, treatments and procedures.
• Monitor client conditions and report changes to Clinical team.
• Collaborate with physicians and agency staff to deliver the best care possible.
• Document observations, interventions, and evaluations pertaining to patient care management.
• Initiate and administer IV medications safely per established protocols.

Part time Registered Nurse Case Manager

Anticipated Territory : Marin Area

Schedule: Must be available Full Days Every Saturday and Sunday (4 SOCs/day)

Also willing to consider Full Time: Saturday and Sunday plus 3 week days.

What you’ll do:

• Conduct Start of Care (SOC) Admissions - OASIS
• Conduct follow ups, evaluation visits as needed in designated territory
• Make home visits to patients in designated territory
• Perform assigned duties, including reconciliation of medication, wound care, treatments, and procedures
• Monitor clients’ conditions, reporting changes to Clinical team
• Collaborate with physicians and agency staff to deliver the best care possible
• Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided.
• Initiate and Administer IV medication (antibiotics and Diuretics) infusions safely and effectively in accordance with established protocols.

To be a success:
