Qualifications

• Current RN license in the state of practice.
• Compact licensees must transfer their license within 60 days.
• BSN from an accredited institution; education verified.
• BLS for Healthcare Providers required.
• RNs with <12 months experience must obtain BSN within 4 years of hire.

Responsibilities

• Coordinate care as navigator for palliative patients and families.
• Educate patients and families on diagnosis, treatment options and resources.
• Assess symptoms and collaborate on symptom management plans.
• Triage patient and family calls and concerns.
• Coordinate services across primary, specialty, hospice, home health, and community resources.
• Support advance care planning conversations and documentation.
• Provide ongoing follow-up and care coordination throughout the journey.
