Job summary

RenalCare Associates S C in Peoria, IL seeks an experienced Licensed Practical Nurse (LPN) to join our outpatient clinic. You’ll apply strong nursing skills, critical thinking and clear communication to assess patient needs and coordinate care.

This Monday–Friday position has no nights, weekends or holidays. You’ll triage calls, prioritize issues, document in the EMR and educate patients while collaborating closely with physicians and the clinical team.

Qualifications

• Current Illinois LPN license.
• Professional nursing experience.
• Strong critical thinking and clinical decision-making.
• Ability to prioritize patient needs and respond appropriately.
• Clear, professional communication with patients and staff.
• Organized, dependable and detail-oriented.
• Willingness to learn new clinical information.

Responsibilities

• Triage patient calls and assess symptoms.
• Prioritize urgent vs routine issues.
• Communicate changes to the physician.
• Coordinate physician orders, medications, testing, referrals and follow-up.
• Review clinical information and ensure action and follow-through.
• Provide patient education on medications and care plans.
