Position Summary:

As an Independent Optometrist leasing space inside Walmart stores, you will play a leading role in patient eye and vision care, as well as general health and well-being. You will have the autonomy to:

1. Establish your practice days and hours
2. Control your patient schedule
3. Set your fee schedule
4. Keep 100% of exam and professional fees

Optional support services available at fair market value include equipment/instrumentation and administrative or opt-tech support (if permitted by state).

Minimum Qualifications for a lease agreement:

• Doctor of Optometry (OD) from an accredited optometry school
• Valid state optometry license
• Ophthalmologist (MD or DO) from an accredited school
• Valid state ophthalmology license
• General Liability Certificate
• Professional Liability Insurance Certificate
• W-9 form
